I agree. The lower you can resolve a complaint, the better. It costs less to both the company and the customer. But when you get caught up in that good old "policy loop," don't be afraid to call someone a little higher up the chain. As a district manager, I get a lot of these calls, but they're almost always an opportunity to teach an employee about the big picture. Customer's have a lifetime value. Don't let a $2.99 product prevent you from getting a $50,000 future sale!

I work for an utility company, and I heavily underline tip #1. But give the first level help a try before asking for a manager. We honestly deal with your type of problems day in and out, and often if you do get our manager (who manages people and numbers), they have to come back for us to have us do what I would have done in the first place. If you're unhappy and you want a second opinion, by all means ask for a manager.
Also, for utilities there's always the PUC (Public Utility Commission).

My wife has been known to complain by writing letters. She's gotten so many free coupons for things that I've lost count. She gets free coupons for household items when they don't perform as well as she wanted, coupons for free things when the cereal box wasn't as full as she would have liked, or the lipstick broke off, or anytime something didn't go just right. Complaining is effective, especially when you write a snail mail letter.
